Axsome Beats Handily as Auvelity Growth Accelerates, but a Battered Stock Shows Investors Still Doubt the Ramp

Axsome Therapeutics delivered the kind of quarter its shareholders had been waiting for and the market had been pricing out. The company reported a third-quarter loss of $0.25 per share, well ahead of the -$0.69 consensus, a 63.8% beat and a 73.4% improvement from last year's -$0.94. Revenue of $259.69 million topped the $246.45 million estimate by 5.4% and grew 51.9% year over year. That growth rate is faster than the 46% pace posted in the second quarter, and sequential revenue rose 19%, pushing Axsome past a $1 billion annual run rate. The central tension is that fundamentals are accelerating just as the stock has been unwinding. Shares are down roughly a third since the last report, and management declined to endorse fourth-quarter or 2027 consensus.

The quality of the beat holds up better than the stock's behavior would suggest, though it deserves some scrutiny. SUNOSI's 47% growth was flattered by an $8.5 million ex-U.S. milestone. Even so, total revenue would still have cleared consensus without it. The operating story is genuine. SG&A was essentially flat sequentially at $203.1 million versus $208.1 million in Q2, as management had signaled, while revenue jumped. That drove the net loss down to $12.9 million from $51.3 million in the prior quarter. Two caveats apply. First, a $2.4 million gain on contingent consideration helped the bottom line, compared with a $13.2 million charge a year ago. Second, accounts receivable swelled to $343 million from $224 million at year-end. Cash slipped only modestly, to $313.9 million, and management reiterated that current cash should carry the company to cash-flow positivity.

AUVELITY is doing the heavy lifting. Net sales reached $207.6 million, up 53% year over year and 15% sequentially. Total prescriptions of roughly 302,000 rose 14% from Q2, and new-to-brand prescriptions climbed 21% sequentially. The Alzheimer's disease agitation launch is now visible in the numbers. Among patients 65 and older, new-to-brand prescriptions rose 211% year over year and 99% sequentially, and monthly prescriptions in that group have climbed to about 14,800 from 8,300 before launch. The expanded salesforce is converting too. New prescribers hit a record of about 8,200, up 49% sequentially, and under-65 monthly volume is up to roughly 88,200 from 67,000 before the expansion. Pricing also helped, with AUVELITY gross-to-net improving to the mid-to-high 40% range from about 50% in Q2.

Bears still have legitimate ammunition:

  • **SYMBRAVO is stalling.** Sequential prescription growth slowed to 7% from 30%, and payer coverage remains stuck near 57%.
  • **SUNOSI's underlying growth is easing.** Sequential prescription growth slowed to 6% from 8%.
  • **Gross-to-net will reverse slightly.** Management expects discounts to tick up in Q4 on seasonality and guided 2027 to look similar to 2026.
  • **Medicare friction.** Prescription abandonment is higher in Medicare than in commercial for the Alzheimer's agitation indication.
  • **Weekly data are getting noisier.** IQVIA capture fell to roughly 90%, and a pharmacy chain stopped reporting to one data provider in October, which will make prescription trends harder to read.
  • **Pipeline timing slipped again.** The binge eating disorder readout moved to 1Q27, after earlier targets of 2H26 and then Q4 2026.

The catalyst calendar is dense:

  • **1Q27:** ENGAGE topline results in binge eating disorder.
  • **May 1, 2027:** PDUFA date for AXS-12 in cataplexy, with no advisory committee planned and pre-launch work underway.
  • **1H27:** Starts of the AXS-20 Phase 3 trial in schizophrenia and AXS-17 trials in epilepsy.
  • **2H27:** Readouts from SUMMIT in smoking cessation and multiple solriamfetol trials.

The market setup is more complicated than the surprise implies. The stock entered the report at $144.43, 33.1% below the $215.88 open after the August release. That leaves it 37.1% below its September 11 inter-earnings high of $229.62 and 27.5% under its 200-day moving average. Analysts on the call noted the shares are down 40-45% since the agitation launch. The stock was sitting only about 10% above a quarter low set days before the report. Investor sentiment remained negative but improved meaningfully, rising from -0.43 to -0.06. That suggests expectations had already been reset lower heading into the print. The Earnings Whispers trend signals are mixed, with sequential growth and momentum positive while price and AVWAP readings remain negative.

The bottom line is that Axsome produced accelerating revenue, a sharply narrower loss, and clear evidence that the AUVELITY expansion is working, which is precisely the outcome turnaround skeptics were not positioned for. The beat was not perfect: a one-time milestone, a fair-value gain, softening secondary brands, and management's refusal to back consensus all leave room for doubt. But the core franchise is gaining speed while the stock trades near its lows, and that gap between fundamentals and price is now the story investors must resolve.
