Neurocrine Crushes Estimates and Raises INGREZZA Again, but VYKAT XR Doubts and a Softer Q4 Cloud the Record Quarter

Neurocrine Biosciences delivered one of the cleaner beats of the season on the surface. Non-GAAP earnings of $3.29 per share topped the $2.57 consensus by 32.1% and cleared the more cautious $2.49 Earnings Whisper number by an even wider margin. Revenue of $1.123 billion beat the $1.04 billion consensus by 8.0%. Earnings grew 61.3% year over year and revenue rose 41.2%, and net product sales crossed $1 billion in a quarter for the first time. Management also raised INGREZZA guidance for the second straight quarter. Yet the call introduced a genuine crack in the story. VYKAT XR, the asset that justified the Soleno acquisition, now faces a slower path back to growth amid safety scrutiny. A quarter that should have been a victory lap instead leaves investors weighing a strengthening core against a wobbling new growth leg. That is the central tension.

The quality of the beat deserves some scrutiny, though it largely holds up. On a GAAP basis, diluted EPS actually slipped to $1.96 from $2.04. Three items drove the gap between GAAP and non-GAAP:

  • a $52 million unrealized loss on equity investments, versus a $31 million gain a year ago;
  • $35 million of amortization tied to the roughly $2.2 billion of VYKAT XR intellectual property;
  • Soleno-related compensation and integration costs.

These are largely non-operating or acquisition-accounting charges rather than evidence of eroding economics. Operating income still rose to $331 million from $239 million. Non-GAAP R&D climbed to $317 million, reflecting Phase 3 programs for osavampator and direclidine. Non-GAAP SG&A rose to $327 million. Even so, management trimmed full-year non-GAAP SG&A guidance to $1.29-$1.31 billion from $1.325-$1.35 billion, a sign of operating leverage arriving faster than planned.

The core franchise is accelerating, not merely holding. INGREZZA sales stepped from $657 million in Q1 to $716 million in Q2 and $801 million in Q3. Management cited record new and total prescriptions and high-teens TRx growth for 2026. Full-year guidance now stands at $2.9-$2.925 billion, up from $2.7-$2.8 billion entering the year. CRENESSITY built steadily from $153 million to $184 million to $211 million. It now reaches more than 15% of diagnosed classic CAH patients, with roughly 80% reimbursement on dispensed prescriptions. Early 2027 commentary was constructive as well. Management expects INGREZZA formulary access at least as good as 2026, with double-digit volume growth more than offsetting price concessions.

Bears, however, have legitimate ammunition:

  • **INGREZZA timing:** Q3 sales included a roughly $20 million order-timing benefit that is expected to reverse. Even the top end of guidance implies a sequentially lower fourth quarter, which helps explain the note that the company's outlook sits below revenue estimates despite the raised range.
  • **Pricing:** The 2027 INGREZZA net price concession is expected to deepen to mid-to-high single digits from about 4% in 2026, ahead of IRA price-setting in 2029.
  • **VYKAT XR:** This is the bigger issue. Sales did reach $107 million, up from $94 million pro forma in Q2. But management now concedes the return to growth "might be a little bit more delayed" because of "recent external events." Analysts pressed on rising FAERS adverse-event counts and a post-grant review instituted on a VYKAT XR patent. That marks a clear retreat from the steady sequential growth framing offered last quarter.

On the pipeline, management highlighted record R&D activity. It cited the NBI-567 Phase 2 start in Alzheimer's cognition and a triple-agonist obesity program entering Phase 1. Direclidine's Phase 3 is running with a discontinuation rate of 3% or less. NBI-986, by contrast, was discontinued. The December 15 R&D Day and 2027 Phase 3 readouts are the next major catalysts. Cash has rebuilt to roughly $790 million from about $500 million after the Soleno close, supported by $400 million of operating cash flow even after around $100 million of buybacks.

The market had already turned skeptical before the print. Shares closed at $146.10 heading into the report, down 20.7% from the $184.34 open following the July release. That left them about 21% below the inter-earnings high set that same day, roughly 10.5% above the October 14 low, and sitting almost exactly on the 200-day moving average. Investor sentiment slid from a marginally positive 0.05 to -0.17, a meaningful swing into negative territory. That shift is consistent with growing VYKAT XR anxiety. The Earnings Whispers trend signals are split. Sequential growth and momentum readings are positive, while price and AVWAP trends remain negative, mirroring the gap between improving fundamentals and deteriorating stock behavior.

The bottom line is that Neurocrine's core business is performing better than the stock implies. Accelerating INGREZZA and CRENESSITY sales, a second guidance raise, and expanding operating leverage give the bull case real substance. But the softer VYKAT XR outlook, a timing-inflated Q3, a sequentially weaker Q4, and steeper 2027 pricing explain why sentiment soured. The burden now shifts to the R&D Day and VYKAT XR's next few quarters to show that the safety noise is a delay rather than a derailment.
